Integrated community paediatric services


The service will provide medical and nursing holistic care for children & young people who require assessment, intervention, advice and support to manage short or long term health conditions, who may require community based secondary care and sudden worsening (acute exacerbation) associated with those conditions.  The service will promote the health and wellbeing of all children and young people, working where appropriate with other health and social care partners to assess and plan care, wrapping services around the child/young person.

The service will ensure children & young people’s needs are managed effectively to reduce the demand on urgent care systems, supporting early discharge and deflection away, from hospital where appropriate.

How we meet your needs

Our service offers support and assessment in a range of settings.  If your child or young person attends special school or mainstream school, in Bolton, we may be able see them in their educational setting.  We provide medical and nursing clinics throughout the Bolton locality but can also facilitate home visits where this may be more appropriate relating to age, condition or family circumstances within a time frame agreed by family.   Where home visits may not be appropriate, we attempt to appoint clinic appointments locally for families or they may have a choice of which clinic to attend where phlebotomy (blood samples) intervention is required.

Palliative Care

The Integrated Community Paediatric Service, Continuing Care Team support children and young people at the end of life, whose preferred place of care is at home.

The team work flexibly depending on the needs of the individual, and can help with medications, symptom management, and advanced care plans.  They offer bereavement follow up visits and signposting for further support. 

In addition, Derian House provide respite and end-of-life care to children, young people and their families from Bolton.
