The 0-19/25 Public Health Nursing Service brings together Specialist Community Public Health Nurses (Health Visitors and School Nurses), who are supported by Public Health Nurses and Public Health Assistant Practitioners, to offer a seamless family centred service to all children and young people, who either live in Bolton, or who attend formal education in Bolton.

The service is initially offered from 28 weeks of pregnancy, continuing right through until the end of formal education, this can be extended to the age of 25, for young people with additional needs, or who are being supported by the Leaving Care Team.

You can expect to see your health visitor at:

  • 28-38 weeks antenatal
  • 10-14 days post delivery
  • 6-8 weeks post delivery
  • 1 year of age for the child
  • 2 2.5 years of age for the child.

You can expect to have contact form your school nurse for your child to be offered:

  • school entry health assessment
  • A health assessment in year 6
  • A health assessment in year 9

We have an immunisation team that goes into school and delivers vaccinations following parental consent, at:

  • Year 9 Human Papilloma Virus, 2 doses
  • Year 10 School leaver booster Diphtheria, Tetanus and Polio

We also have an enhancing families nursing team:

  • This team is made up of nurses with advanced skills in communication and evidenced based techniques to support families in improving health and developmental outcomes. You can access this service through your health visitor or school nurse.

How we meet your needs

The 0-19 Public health nursing service works with the whole family to identify health needs and to promote healthy lifestyles and will then work in conjunction with local partner agencies, including local voluntary agencies, to develop a tailored package of care, keeping the needs of the child/young person at the centre of any interventions.

There are nine community based teams covering a defined geographical area, some are based in health centres and some within the local Startwell centres.
