D4 Medical Examination Report form updated - Taxi trade updates

The Licensing Team is writing to inform you that the D4 Medical Examination Report form was updated on 27 February 2026.

 The original email you received regarding the D4 form included a link that takes you directly to the official website where the DVLA publishes all current and updated versions of its forms. This link will always take you to the latest version available. Please see link below:

 Please ensure that you use the original link provided when you need to download the D4 form, do not download the form in advance. This is essential to ensure you are using the most up‑to‑date version of the form.

 You must make sure your doctor completes the correct and current version of the form. On the medical declaration the doctor must confirm they have completed the most up-to-date version of the D4 Medical Form.

 Using an outdated version will result in your application being rejected by the Licensing Team.
