Chaperone service 

 

Persons are licensed under the Children (Performances) Regulations 1968 by the Local Authority (LA) to act as Chaperones. They are subject to an enhanced Criminal Records Bureau (CRB) check.

Once the application has been approved the licence is valid for 3 years. The Council will hold the person’s details but they are NOT employed by the Council, nor will the Council find them work or recommend them to any prospective employer. The Council will respond to any enquiry from prospective employers by supplying details of all persons licensed as a Chaperone.

Applications to act as a Chaperone must include the following documents:

An application form for an enhanced CRB check – accurately completed as far as possible;

Two recent passport sized photographs, authenticated on the reverse by an independent person who knows the applicant;

Two character references from independent persons;

Two proofs of residence e.g. Utility Bill; Council Tax Bill; Bank or Credit Card Statement or similar (which will be returned) and

Copies of the following:

Birth certificate;

Marriage certificate (if appropriate);

Passport or Photographic driving licence (if the applicant has one).
